Template, not built for a specific business

Catching the visitor who's comparing options at 9pm

A request form for a business that's currently phone or email only. Not a real-time booking calendar, a captured request that gets confirmed the same way a phone call would. This page uses a fictional clinic to show the mechanism.

Everything below is fictional. When this gets built for a real business, the service list and hours come from their own site, the way the example below shows.

How this gets built for real

A request, not a live booking system. It doesn't know which times are actually free unless it's connected to the business's real schedule.
Framed as additive, not as fixing something broken. A phone-only model can be a deliberate, working choice. This only catches the person who's still deciding and finds the site outside open hours.
