Working example, built for Baxter Appliance Repair

Catching the appliance details, and the warranty question, before the phone even rings

Two other appliance repair shops serving Greencastle, Sears Home Services and All-Budget Appliance Repair, already let customers book online. Baxter's site still routes everyone through a phone call or a basic contact form, and separately has to tell people on the phone "we do not do warranty work." This form asks the warranty question up front, and only takes appliance type, brand, model and symptom for the jobs Baxter would actually take.

What's real: the appliance categories, the warranty policy ("we do not do warranty work"), and the service area are copied from Baxter's own site today. What's illustrative: the specific brands and models in the confirmation screen below are sample entries, not real customer submissions.

What I'd flag before this goes live

The warranty question is the whole point. Right now that question gets asked and answered on a phone call. Here it's asked first, so a warranty job never becomes a wasted trip or callback, it just gets a clear answer and a pointer to the manufacturer instead.
This is a request form, not a live booking calendar. It doesn't know which specific times are actually free, because it isn't connected to a real schedule. A customer submits appliance details and a rough preference, and it reaches Baxter the same way a phone call would, just with the diagnostic details already written down.
Nothing here actually sends anything. A real version would text or email Justin and Alyssa the moment someone submits, with the appliance type, brand, model and symptom already filled in before they call back. This demo just shows what that submission would look like.
Missed-call text-back is a separate, paired idea, not built into this page. The original idea was this intake form plus an automatic text reply when a call goes to voicemail mid-repair, pointing the caller to this same form. That's a phone-system integration and isn't something a static page can demonstrate, illustrative only.
Appliance categories and the warranty line are copied from Baxter's site today. If either changes, this needs a one-line update to match, it won't drift out of sync on its own.
