Working example, built for Ask a Luthier

Giving the repair shop a front door of its own

Right now, alferdpackerband.com mentions that Steve is a luthier, but there's no link from there to ask-a-luthier.com, and ask-a-luthier.com itself has a Contact link with no request form behind it - just phone or email tag. Someone who lands on the band site looking for a repair, or reaches the luthier site after hours, has no way to leave a request. This form is that missing piece.

What's real: the repair categories below (neck resets, truss rod work, fret jobs, bridge and saddle replacement, finish work) are drawn from the questions Steve has answered on ask-a-luthier.com's own Q&A archive, and the instrument types match what he covers there - guitars, mandolins, violins, cellos, and basses. What's illustrative: the site doesn't list hours, a shop address, or turnaround times anywhere, so those below are placeholder values marked "Illustrative" - a real build would use whatever Steve actually runs on.

The gap this closes

The band's own site, alferdpackerband.com, names Steve as a luthier but sends nobody anywhere. His shop site, ask-a-luthier.com, has a Contact link in the nav with no way to actually request anything behind it. Two real people looking for the same thing - a bandmate's fan curious about the repair work, and a musician searching for a luthier - both hit a dead end today. Linking the two sites and dropping this form where "Contact" currently sits fixes both at once, without changing how Steve actually works.

What I'd flag before this goes live

This is a request form, not a booking calendar. It doesn't know which days Steve is actually free, because it isn't connected to a real schedule. Someone picks a rough preference and it gets sent to him, same as a phone call or email today, just captured from both sites instead of neither.
Hours, turnaround, and shop location are placeholders. Illustrative only, since neither alferdpackerband.com nor ask-a-luthier.com lists them anywhere public. A real build swaps these for whatever Steve actually runs on - even "call to check" is fine, it just needs to be there.
Nothing here actually sends anything. A real version would email or text Steve the moment someone submits it. This demo just shows what that submission would look like.
The fix is linking two existing sites, not replacing either. The band site keeps being the band site. This form slots into the Contact page on ask-a-luthier.com, and a single line on alferdpackerband.com points there.
